Membership Information

As a Private Club, Memebership is a must. You will be granted membership with a valid club card in good standing from any other compassion club. You must obtain a Section 56 membership card & you must bring the following information.

• Current Photo ID
• Doctors' name, address & phone number
• Letter of diagnosis

Please complete your membership application. You must sign appliation form and return it signed by your doctor as well. We require letter of diagnosis from your doctor which specifies your medical condition. This will, of course, be confirmed by us PRIOR to granting membership status. There is no such thing as INSTANT MEMBERSHIP.

This membership is subjected to terms and conditions and may be revoked and suspended without prior notice for violation of club rules and procedues. Please contact us for details.
